We cannot accept different names without any proof of legal name change. You can specify initials instead of first or middle names, for example MICHAEL H JOHNSON. For Asian Christian names, we can accept these if the main names are the same e.g. JOHN TAN LAY SENG, where 'John' is the popular unofficial name used.

Note: Airmail €30. Courier €65. Testamurs will not be released until all payments are made and all debts honoured. After submission, you will be redirected to the Warnborough Online Shop to make your payment.

PLEASE READ AND UNDERSTAND BEFORE SUBMITTING THIS FORM

This form should be submitted at least four weeks before the date of graduation. You will be notified of your eligibility to graduate with your results in the final term. Official transcripts may be requested to be sent to other institutions from Euro€10.00 per transcript. Students can apply for a Statement of Completion if evidence of programme completion is required for employment or other purposes. A form for this is available and there is a charge of Euro€10.00.

Change of address:

You must notify us in writing immediately if you change addresses before your graduation ceremony.
